| Exorbitant transportation cost |
George* is an older gentleman who was recently diagnosed with End Stage Renal Disease and hospitalized. As I began to work with him, I learned that he was being transported to the local dialysis unit at an exorbitant cost. To make matters worse, this transportation company was often late. Working together with another Renaissance case manager, I arranged for a different transportation company that charges considerably less and is more reliable, to take George to dialysis. Through my work with George and his new physical therapist, we're hoping that he will eventually be able to travel in a wheel chair via county transportation, which is free of charge. |
| Serious complications avoided |
During a routine follow-up call to Sandra*, it was clear that she was not feeling well, had no appetite and was coughing frequently. I asked if she had called her PCP and she said no, because he was out of the country. I then asked if she had called her nephrologist and she responded "no." I called his office and they paged her nephrologist immediately. As soon as he learned about her symptoms, he scheduled a meeting at her dialysis unit. The care team assessed her condition and addressed her various concerns. Working together, we helped Sandra receive early treatment and avoid serious complications. |
| Grateful patient and family |
I called Candace* for a routine post-hospital follow-up and found her concerned because she was paying too much for transportation to and from her dialysis treatment. She normally drives herself, but after her recent hospitalization, was not yet strong enough to do so. Her son had checked with a local municipal transportation program, but was told she did not live in the "catchment area." Candace was confused because she had seen the vehicles in her neighborhood. I called the program office to inquire about her eligibility. They told me Candace was indeed in the catchment area and that it would cost her $1.70 each way. The patient and her family were grateful for the information. |
