Renal Disease Care Managers
Renaissance Health Care, Inc. offers exciting care management opportunities for the right Registered Nurses. We are seeking experienced nephrology nurses and/or care managers who want to make a difference in patients' lives. Our care managers provide on-site and telephonic care management in various states and regions throughout the United States. At least three years of recent dialysis experience is required. BSN/CNN preferred.

Per Diem Dialysis Nurse
JOB DUTIES: The Per Diem Dialysis Nurse supports the call center nurse. The call center nurse calls the patients while the Per Diem Dialysis Nurse meets with patients face to face to complete disease management assessments that are common among ESRD patients. Produce timely workup of members for vascular access management, dialysis services, co-morbidity management, member education, and hospital and organ transplantation, as appropriate.
